STR Pre-Scheduling Questionnaire

Please complete the following questionnaire and return to STR within 7 business days to allow for the necessary audit arrangements on behalf of Wal-Mart Stores, Inc.  In the event STR does not receive this completed questionnaire within 7 business days, we will advise Wal-Mart Stores, Inc. and STR will not be held responsible for any cost incurred related to denied access due to lack of factory response.  Your cooperation is greatly appreciated.

4. Is there more than one factory under the business license of the factory listed in number 3 above? If yes, please advise each factory covered under one license, address of each, and the distances from your factory address above. 5. Are there any warehouses or production processes being carried outside of the address provided? If no, please proceed to number 7.  工厂在上述地址之外有无其他仓库或车间？如无，请跳过到第7条问题。

a. Does the warehouse or production process belong to the same business license as that of factory?  这些仓库或车间是否与贵厂共用同一营业执照？

b. What is the distance of such processes/department from the current factory premises (approx travel time)?  这些仓库或车间离贵厂的距离（大概车程时间）？
